Objetivo: Caracterizar as notificações de infecção por SARS-CoV-2 em gestantes em um município do sul de Santa Catarina. Métodos: Estudo transversal que avaliou as notificações de casos suspeitos de infecção por SARS-CoV-2 em gestantes no município de Tubarão, Santa Catarina, de março de 2020 a outubro de 2021. Coletaram-se os dados das fichas de notificação de infecção por SARS-CoV-2 da Fundação Municipal de Saúde e no Prontuário Eletrônico do Cidadão (PEC) das gestantes notificadas. A comparação da prevalência de confirmação segundo sintomas apresentados e características maternas foi realizada pela razão de prevalência (RP), com intervalo de confiança (IC) de 95%. Resultados: Foram registradas 555 notificações de gestantes suspeitas para a COVID-19, correspondentes a 487 mulheres. A prevalência de confirmação para a doença foi de 27,3%. O sintoma mais frequente no momento da notificação foi cefaleia (53,0%), entretanto o mais associado à confirmação foi a anosmia (RP: 2,28; IC 95%: 1,68-3,09). Das notificações registradas, 35,0% foram realizadas por gestantes que tiveram contato com casos suspeitos ou confirmados de COVID-19. O contato prévio foi mais relatado por gestantes assintomáticas, em comparação às gestantes que apresentaram sintomas (RP: 1,46; IC 95%: 1,12-1,91). Mulheres que relataram contato prévio com suspeitos ou confirmados para a COVID-19 apresentaram maior frequência de doença confirmada, quando comparadas às gestantes não expostas (RP: 1,80; IC 95%: 1,35-2,39). Conclusão: As gestantes, por terem maior susceptibilidade a surtos de doenças e gravidade do quadro, podem ter adotado com mais atenção medidas como a realização de testes diagnósticos quando em contato prévio com casos de COVID-19, mesmo assintomáticas.


Objective: To characterize the notifications of SARS-CoV-2 infection in pregnant women in a city in the South of Santa Catarina. Methods: Cross-sectional study that evaluated notifications of suspected cases of SARS-CoV-2 infection in pregnant women in the municipality of Tubarão, Santa Catarina, from March 2020 to October 2021. Data were collected from the SARS-CoV-2 infection notification forms from the Municipal Health Foundation, and from the Citizen's Electronic Health Record of the notified pregnant women. The comparison of the prevalence of confirmation according to symptoms and maternal characteristics was performed using the Prevalence Ratio (PR), with a confidence interval (CI) of 95%. Results: 555 notifications of suspected pregnant women for COVID-19 were registered, corresponding to 487 women. The prevalence of confirmation for the disease was 27.3%. The most frequent symptom at the time of notification was headache (53.0%), however, the most associated with confirmation was anosmia (PR: 2.28; 95% CI: 1.68-3.09). Of the notifications registered, 35.0% were made by pregnant women who had contact with suspected or confirmed cases of COVID-19. The previous contact was more frequently reported by asymptomatic pregnant women compared to pregnant women who had symptoms (PR: 1.46; 95% CI: 1.12-1.91). Women who reported previous contact with suspected or confirmed COVID-19 had a higher frequency of confirmed disease when compared to unexposed pregnant women (PR: 1.80; 95% CI: 1.35-2.39). Conclusion: Pregnant women, due to their greater susceptibility to disease outbreaks and disease severity, may have adopted more attentive measures such as diagnostic testing in the face of previous contact with cases of COVID-19, even if asymptomatic.

Abstract Background Psoriasis is a skin disease that affects 1.3% of Brazilians. The use of teledermatology (TD) in the public health sector has democratized access to dermatological care. Objective To analyze TD exams with suspected and/or diagnosed psoriasis performed between 2016‒2020 in the state of Santa Catarina (SC). Methods Analytical cross-sectional observational study that used secondary data collected from the records of TD exams from the Telemedicine and Telehealth System (TTS) of SC. The associations were evaluated by the chi-square test and Student'st test. The significance level was set at 5% (p < 0.05). Results During the period, 6,146 TD exams were related to psoriasis, 58% due to the diagnosis provided by the reporting dermatologist and 42% exclusively due to the suspected disease on the request of the examination. The male sex predominated among the diagnoses of dermatosis (51%; p < 0.001). Regarding risk classification, psoriasis diagnoses were predominantly yellow (58.7%; p < 0.001) or blue (39.7%; p < 0.001) risk, respectively indicating moderate to severe psoriasis (referral to tertiary care) and mild psoriasis (treatment in the primary health care [PHC] level). True positive tests, suspected by PHC and diagnosed with psoriasis through TD, were 34.1% (p < 0.001). Study limitations The TD service is available only for the public health network and analysis for a limited period (five years). Conclusions Psoriasis diagnoses performed by TD, when compared to other dermatoses, tend to receive treatment at the primary (p < 0.001) or tertiary (p < 0.001) health care level, reducing the number of referrals to the secondary care level.

A disponibilização em massa de vacinas contra o vírus SARS-CoV-2 é resultado de esforços científicos mundiais. Entretanto, a insegurança e a hesitação popular per-meiam os movimentos antivacinais. Objetivo: Analisar o perfil dos Eventos Adver-sos Pós-Vacinais (EAPV) na cidade de Tubarão-SC no ano de 2021. Metodologia: Estudo epidemiológico transversal com dados de todas as fichas de notificação padrão de EAPV no município de Tubarão em 2021. As variáveis analisadas foram o perfil epidemiológico do paciente, imunobiológico administrado, o tipo de evento e a evolução do caso. Calculou-se a taxa de incidência (TI) para 10 mil doses apli-cadas. Resultados: A população em estudo foi de 274 pacientes, sendo 73% do sexo feminino, com média das idades de 39,8±14,5 anos. Foram aplicadas 197.001 doses no ano de 2021, o que resultou em uma TI geral de EAPV de 13,9. Houve 206 reações notificadas em pacientes imunizados com a AstraZeneca (TI=29,1), 43 com a Pfizer (TI=5,1), 18 com a Coronavac (TI=4,8) e sete com a Janssen (TI=13,8). Houve apenas sete casos classificados como graves (TI=0,3) e, destes, um paciente evoluiu com óbito, sem relação causal estabelecida. Conclusão: A incidência geral de eventos graves foi baixa, o que corrobora o perfil de segurança dos imunobiológicos disponíveis contra a COVID-19 (AU).


The mass availability of vaccines against the SARS-CoV-2 virus is the result of worldwide scientific efforts. However, insecurity and popular hesitation permeate the antivaccine movements. Objective: Analyze the profile of Post-Vaccine Adverse Events (PVAE) in the city of Tubarão, SC, Brazil, in 2021. Methodology: Cross-sectional epidemiological study with data from all standard PVAE notification forms in the city of Tubarão-SC in 2021. The variables analyzed were the epidemiological profile of the patient, the immunobiological administered, the type of event and the evolution of the cases. The incidence rate (IR) was calculated for 10,000 doses ap-plied. Results: The study population was 274 patients, 73% were female, with a mean age of 39.8±14.5 years. A total of 197,001 doses were applied in the year 2021, resulting in an overall IR of 13.9. There were 206 reactions reported in pa-tients immunized with AstraZeneca (IR=29.1), 43 with Pfizer (IR=5.1), 18 with Coro-navac (IR=4.8) and seven with Janssen (IR=13.8). There were only seven cases classified as severe (IR=0.3) and, of these, one patient died, with no causal rela-tionship established. Conclusion: The overall incidence of serious events was low, which corroborates the safety profile of available immunobiologicals against COVID-19

OBJETIVE: To analyze the impact of maternal vaccination coverage with diphtheria-tetanus-acellular pertussis (Tdap) adsorbed vaccine for adults on pertussis incidence in children under 1 year old in Brazil from 2008 to 2018. METHODS: This was a descriptive ecological temporal trend study using data from surveillance systems managed by the Brazilian National Health System Information Technology Department (DATASUS). Incidence rates and incidence rate ratios (IRR) were calculated with respective 95% confidence intervals (95%CI). RESULTS: There were 20,650 pertussis cases in the study period. In the post-vaccination period there was a 26.6% reduction (IRR = 0.73; 95%CI 0.66;0.82) in pertussis incidence among children aged from 3 months up to but not including 1 year old, and a 63.6% reduction (IRR = 0.36; 95%CI 0.15;0.58) among children from birth to 2 months old. CONCLUSION: The increase in maternal Tdap vaccination coverage coincided with a reduction in pertussis incidence, especially in the birth to two-month-old age group.

Introduction: People living with the human immunodeficiency virus (HIV) are generally overweight or have an altered body composition as compared to healthy individuals, showing a change in nutritional profile over time. Objective: The aim of the study was to characterize the nutritional status, estimate the prevalence of lipodystrophy, and examine the association between lipohypertrophy and lipid profile alterations, and other clinical data of HIV-infected individuals. Methods: This was a cross-sectional study on male and female subjects living with HIV, treated at a specialized outpatient clinic, aged 18 years old and over, whether using the antiretroviral therapy or not. Results: The sample consisted of 420 people with a mean age of 43.8 years (standard deviation 11.7). The length of time of the HIV infection averaged 74.6 months, and 91% of the respondents were on antiretroviral therapy. Lipodystrophy prevalence was 35.7%. Of these, 82 (54.7%) presented lipohypertrophy, 61 (40.7%) had lipoatrophy and 7 (4.6%) had a mixed syndrome. Female gender, body mass index, fat percentage, waist circumference and waist-hip ratio were positively associated with the presence of lipohypertrophy (p<0.001). High mean total cholesterol (p=0.015) and LDL fraction (p=0.028) also showed a statistically significant association with lipohypertrophy. The sampled participants had a nutritional profile compatible with overweight or obesity. No association was found between lipohypertrophy and ART and the therapy duration. Conclusion: Considering the consequences of overweight as a cause of various pathological conditions, preventive measures and interventions are highly recommended for this population.


As pessoas que vivem com o HIV geralmente têm excesso de peso ou composição corporal alterada em relação aos indivíduos saudáveis, apresentando uma mudança no perfil nutricional ao longo do tempo. Objetivo: O objetivo do estudo foi caracterizar o estado nutricional, estimar a prevalência de lipodistrofia e examinar a associação entre lipohipertrofia e alterações no perfil lipídico e outros dados clínicos de indivíduos infectados pelo HIV. Métodos: Estudo transversal com indivíduos dos sexos masculino e feminino vivendo com HIV, atendidos em ambulatório especializado, com idade igual ou superior a 18 anos, em uso ou não de terapia antirretroviral. Resultados: A amostra foi composta de 420 pessoas com média de idade de 43,8 anos (desvio padrão 11,7). O tempo de infecção pelo HIV foi em média de 74,6 meses e 91% dos entrevistados estavam em terapia antirretroviral. A prevalência de lipodistrofia foi de 35,7%. Destes, 82 (54,7%) apresentavam lipohipertrofia, 61 (40,7%) lipoatrofia e 7 (4,6%) síndrome mista. Sexo feminino, índice de massa corporal, percentual de gordura, circunferência da cintura e relação cintura-quadril foram positivamente associados à presença de lipohipertrofia (p<0,001). As médias elevadas de colesterol total (p=0,015) e fração LDL (p=0,028) também mostraram associação estatisticamente significante com lipohipertrofia. Os participantes da amostra apresentavam perfil nutricional compatível com sobrepeso ou obesidade. Não foi encontrada associação entre lipohipertrofia e terapia antirretroviral e duração da terapia. Conclusão: Considerando as consequências do excesso de peso como causa de diversas patologias, medidas e intervenções preventivas são altamente recomendadas para essa população.

PURPOSE: This study evaluated the effects of 12 weeks of karate training on cardiometabolic parameters, oxidative stress, and inflammation in adolescents with overweight and obesity. METHOD: Seventy adolescents were randomized into 2 groups: control received nutritional and psychological interventions once a week for 12 weeks, and treatment received nutritional and psychological interventions once a week, plus 3 karate sessions per week, for 12 weeks. The main outcome measure was improvement in cardiometabolic parameters, oxidative stress, and inflammation. RESULTS: After the intervention period, the treatment group showed a reduction in resting heart rate (77.86 [10.89]), high-density lipoprotein cholesterol (40.86 [8.31]), and triglycerides (75.18 [32.29]) and an increase in low-density lipoprotein cholesterol (95.64 [42.53]) in relation to pretraining. Regarding oxidative stress markers, there was a reduction in protein carbonylation (0.07 [0.06]) and nitric oxide (1.39 [1.11]) and an increase in superoxide dismutase (0.68 [0.31]) and glutathione (0.11 [0.08]) compared with pretraining. With respect to inflammation, adiponectin increased (14.54 [5.36]) after the intervention when compared with preintervention. CONCLUSION: The study concluded that the intervention may improve cardiometabolic parameters, oxidative stress, and inflammation in adolescents with overweight and obesity. Long-term effects need to be evaluated.

Objetivo: Analisar o impacto da vacina adsorvida difteria, tétano e coqueluche (pertússis acelular), tipo adulto (dTpa), materna, na incidência de coqueluche em crianças menores de 1 ano no Brasil, de 2008 a 2018. Métodos: Estudo de tipo ecológico, descritivo de tendência temporal, utilizando-se de dados dos sistemas de vigilância do Departamento de Informática do Sistema Único de Saúde (Datasus). Foram calculadas taxas de incidência e razão das taxas de incidência (IRR), com respectivos intervalos de confiança de 95% (IC95%). Resultados: O número de casos de coqueluche foi de 20.650. Houve redução na incidência de coqueluche no período pós-vacinal, de 26,6% (IRR = 0,73; IC95% 0,66;0,82) nas crianças de 3 meses a 1 ano incompleto, e de 63,6% (IRR = 0,36; IC95% 0,15;0,58) nas crianças de zero a 2 meses. Conclusão: O aumento da cobertura da vacinação com dTpa materna correspondeu à redução na incidência de coqueluche, principalmente na faixa etária de zero a 2 meses.


Objetivo: Analizar la incidencia de tos ferina en menores de 1 año y su relación con la cobertura de vacunación materna por la vacuna adsorbida contra la difteria, el tétanos y la tos ferina (tos ferina acelular) tipo adulto (dTpa), de 2008 a 2018, en Brasil. Métodos: Estudio ecológico descriptivo de tendencia temporal utilizando datos de los sistemas de vigilancia del Departamento de Informática do Sistema Único de Saúde (Datasus). Se calcularon los tasas de incidencia y las razones de tasas de incidencia (RTI) con los respectivos intervalos de confianza del 95% (IC95%). Resultados: El número de casos de tos ferina en el período de estudio fue de 20,650. Hubo una reducción en la incidencia de tos ferina en el período post-vacunación. En los niños de 3 meses a 1 año incompleto, la reducción fue del 26,6% (RTI = 0,73; IC95% 0,66;0,82), mientras que en el otro grupo la diferencia relativa fue del 63,6% (RTI = 0,36; IC95% 0,15;0,58)]. Conclusión: La vacunación materna con dTpa coincide con una reducción en la incidencia de tos ferina, especialmente en el grupo de edad de cero a 2 meses.

RESUMO

The use of coronavirus disease 2019 RNA vaccines in pregnant women led to reports on the first cases of newborns with antibodies to sudden acute respiratory syndrome coronavirus 2 (SARS-CoV-2), a phenomenon that was unknown when using immunizations with inactivated viruses. Thus, this study aimed to report a case of passive anti-SARS-CoV-2 immunity in a newborn through immunoprophylaxis of a pregnant woman who received the CoronaVac® vaccine in the third trimester of pregnancy. Twenty-four hours after delivery, samples were collected from the newborn and screened by enzyme immunoassays, which revealed antibodies to SARS-CoV-2.
